By eliminating the need to travel to vaccination clinics, CareSend makes vaccination more accessible while providing personalized, professional care in the comfort and safety of patients' homes.
The Problem
Vaccine administration requires meticulous documentation for every dose administered. Nurses must collect patient consent before vaccination, record detailed information about the vaccine lot, dosage, and administration site, and provide patients with official vaccination records. This documentation serves multiple critical purposes: legal protection through informed consent, public health tracking, and providing patients with proof of vaccination needed for travel, work, or school.
In the context of in-home vaccination, this paperwork becomes particularly challenging. Nurses traveling to patients' homes need to manage physical forms, ensure all required information is captured accurately, and maintain organized records for every patient visit. Manual paperwork in home settings creates risks: forms can be lost, handwriting may be illegible, required fields might be skipped, and transferring handwritten information to digital systems creates additional work and error opportunities.

The Solution
CareSend implemented Anvil's PDF Fill API to automate consent forms and vaccination records, creating a digital-first documentation process for in-home vaccinations.
The workflow begins when a vaccination appointment is scheduled. Patient information—name, date of birth, address, medical history relevant to vaccination—is collected through CareSend's platform. When the nurse arrives for the appointment, this information has already been used to generate a pre-filled consent form through Anvil's PDF Fill API.
The consent form contains all necessary legal language about vaccine risks, benefits, and patient rights, with the patient's personal information already accurately populated. The nurse reviews the form with the patient, answers questions, and collects the patient's signature. No manual transcription is needed, and the nurse can be confident that all patient identifying information is correct.
After the vaccine is administered, the nurse enters administration details—vaccine manufacturer, lot number, dosage, injection site, time of administration—into CareSend's mobile interface. This data immediately flows to Anvil's PDF Fill API, which generates an official vaccination record. The record includes both the patient's information and the specific details of the vaccination, creating a complete, accurate document that can be printed for the patient or delivered digitally.
All generated documents are automatically stored in CareSend's system, creating a complete audit trail and ensuring that vaccination records are securely maintained. The digital workflow eliminates the risk of lost paperwork and ensures that every vaccination is properly documented for public health purposes.
